The Horizons Transition Program is designed specifically for neurodiverse young adults who are ready to explore life beyond high school—but not quite ready to make the leap alone. Whether your child is preparing for college, vocational training, or employment, Horizons offers a personalized bridge to what’s next.
College-Ready, Not Just College-Aged
Students don’t just enroll in college classes, they learn how to be college students. We focus on organization, self-advocacy, and executive functioning to build a strong foundation.
Strengths-Based Support
We don’t "fix" students—we equip them. Through mentorship, goal-setting, and skill-building, students gain confidence and agency in shaping their futures.
Real-World Responsibility
Internships and part-time roles are more than resume builders; they’re chances to apply skills in authentic settings, with built-in reflection and feedback.
Community Connection
Students stay plugged into Hill Top’s supportive, inclusive culture while preparing to transition into adulthood with greater independence.
3 Program Paths
For students ready to begin exploring college while still benefiting from a structured support system.
Includes:
- Landmark College’s “Principles of Learning” course
- Up to 2 college courses per semester
- 1–2 Hill Top classes for skill-building or interest
- College support liaison
- Internship/employment/volunteer placement
- Group & individual counseling
- Participation in Hill Top activities
For students ready to dive deeper into academics and college life.
Includes everything in The Comprehensive College Path, plus:
- Up to 3 college courses per semester
- Expanded support for managing the academic load
For students who need more time to strengthen their academic, social, and life skills before entering the next phase.
Includes:
- Full schedule of Hill Top courses
- Dual enrollment in 1–2 college classes
- Optional on-campus residential experience (additional fee)
- Counseling, social opportunities, and ongoing mentorship

Horizons families can personalize the experience through à la carte enhancements such as:
- Additional college course
- Executive functioning & academic coaching
- PEERS® Social Skills Coaching (14 sessions)
- Individual counseling sessions
- Expanded activity involvement or leadership roles
